The RN Program Coordinator is the primary point person for ensuring the collaborative care of patients from the time of hospital discharge through all follow ups.
They work closely with hospital and other PPG staff to ensure smooth transition of care including testing and clinic visits.
ACCOUNTABILITIES
Gather and review eligible patient’s self-reported health information.
Update the eligible patient’s medical, social and family history.
Review the patient’s Health Risk Assessment to identify potential barriers, risk factors, opportunities for education and gaps in recommended preventive services.
Triage phone calls; coordinate care for patient with provider and other service lines/specialties.
Furnish personalized health advice to the patient and provide referrals as appropriate for recommended testing or preventive counseling.
Update the Electronic Health Record to reflect the patient’s most current information.
Establish and maintain positive working relationships with all key customers (physicians, office staff, patients, families, and interdisciplinary team).
Engage patient in self-management.
Provide timely, clinical documentation of patient encounters, using the EHR and other tools.
Monitor patient’s chart for test results, communicate results to patient in a timely manner.
Coordinate patient’s procedures and possible hospital admissions.
Participate in patient’s clinical appointments, assisting in care of patient.
Review and queue patient’s medications for provider to review and sign.
Complete special projects, as assigned by leadership team.
Additional responsibilities/duties will be assigned based on team work load and competency
Education: Graduation from accredited Nursing Program
Skills: Excellent organizational skills, critical thinking, attention to detail, dependable and motivated.
Must be able to work autonomously.
Working knowledge of office based regulatory requirements.
Problem solving and critical thinking.
Flexibile with hours to meet the needs of department.
The Coordinator must be able to work through the office and hospital based electronic health records.
Demonstrated ability to communicate with medical and administrative staff and patients, excellent understanding levels of care.
Years of Experience: 1 year of relatable or equivalent experience
License: Active Registered Nurse License in state of assigned practices.
Certification: ACLS and BLS certification
PREFERRED QUALIFICATIONS
Education: Bachelor’s of Science in Nursing
Skills: EPIC experience.
Neurology nursing experience preferred
Years of Experience: 2+ years relatable experience
WORKING CONDITIONS
Physical Demands: Frequent Sitting and computer use.
