As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system.
You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients.
You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.
This role will support of clinics in the North Columbus and Toledo markets.
Ideal candidate will live in a location equidistant to both cities.
Position Details Location: Hybrid-Remote.
Work from home and, as needed, will Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.
Complete Comprehensive Health Assessments Manage complex patients, focused on reducing hospitalizations and improved health outcomes Monday- Friday schedule with the ability to accommodate patient and nephrology partner’s availability.
Must be flexible.
Full-Time self-managed schedule Competitive pay and Excellent Benefit package CEU/CME reimbursement + Paid License renewals & many other benefits · Requirements Passion for delivering care to patients with complex, chronic illnesses Minimum of two (2) years' experience as a Nurse Practitioner (NP) Current unrestricted NP license to practice in the state Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification Intermediate computer skills and proficiency in MS Word, Excel & Outlook required Private Home Office, High Speed Internet and Reliable Transportation Required.
Preferred Qualifications Preferred experience working in Value Based Care or Population Health including documenting and coding from an MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes) Manage Population Health (hospitalization) DaVita IKC (Integrated Kidney Care) is comprised of an innovative team who is committed to improving patient outcomes and improving quality of life.
