Charles Hospital - Full Time - Days The Social worker/Counselor assists the Social Worker/Counselor II with planning and coordination of patient care.
Must have considerable knowledge of therapeutic interventions, and age specific needs of both geriatric and adults.
Ability to perform intake/clinical assessments and perform functional assessments.
Ability to schedule appointments.
Facilitate psychoeducational groups.
Plans all aspects of discharge planning including but not limited to outpatient mental health, probate court process, PASRR.
For the licensed chemical dependency counselor, they will be responsible for providing treatment related to co-occurring substance abuse diagnosis both in inpatient and outpatient setting, providing appropriate education and therapy including group therapy.
ESSENTIAL FUNCTIONS (7-10 statements) Provide individual assessments and group treatment in the inpatient setting.
Documents in medical record In accordance with accepted policy in a timely manner.
Licensed Independent chemical dependence counselor may be asked to provide supervision to staff eligible and pursuing this licensure.
Social Worker/Counselor 1 to provide discharge planning, assessments and psychoeducation groups.
Co-facilitate psychotherapy with Social Worker/Counselor II staff if needed.
Works closely with utilization review to faclltate discharge planning.
Displays a curiosity and desire for knowledge and staying current v.
ith colleagues, be curious about business and the health care services industry.
Regularly invests time in encouraging the team to innovate, continuously improve processes and share knowledge.
Maintains the privacy and security of all confidential and protected health information.
Uses and discloses only that information which Is necessary to perform the function of the Job.
Participates in the safe management of the milieu, intervening effectively v.
ith patients who behave inappropriately or who are having coping difficulty, utHizing effective non-abusive crisis prevenlion/interventlon techniques.
Treats patients and families with empathy and respect.
Maintains effective working relationships with team members, (requires both flexibility and setting limits}.
Participates in lhe administrative supervision of crinical interns or students and/or volunteers, utilizing orientation, observalion, record reviews, feedback and evaluations as required.
Performs work in accordance with tha facility standards and policies regarding risk management, safety, infection control, etc.
Performs job functions adhering lo service principles with customer service focus of innovation, service excellence and teamwork lo provide Iha highest quality care and service to our patients, families, co-workers and others.
Develops annual personalized professional goals in conjunction with manager/director.
Seeks and participates in learning opportunities that meet personal and departmental goals.
Other duties as assigned by manager or director.
Faciliates family sessions as needed.
Maintains and collects collateral health information with community partners.
